PREPARATION TIPS FOR CONSULTATION

1. Be on time for your Zoom appointment. 
NB: Video conferencing may be necessary to visualise the presenting complaint.  For visualisation of the mouth/throat, have another light/flashlight on hand if needed.
NOTE THAT NOT ALL COMPLAINTS CAN BE MANAGED VIA TELEMEDICINE.
2. PRESCRIPTION: Be prepared to email or have the following information on hand if your consultation results in the need for a prescription:
  • Name

  • Age

  • Date of Birth

  • Email Address

  • Contact Number

  • Any allergies to food and/or medication

  • Pharmacy Name & Number

3. SICK LEAVE: If your consultation results in the need for sick leave, have the following information on hand:

  • Company Name & Address

  • Any relevant names/email addresses note should be addressed to or copied to.
